Chronic Conditions Management Programme (CCMP)
The Spring Garden Chronic Conditions Management Programme (CCMP) is a long-term programme under various specialities which will provide detailed assessment, treatment and continuous monitoring to make sure that your chronic condition is always taken care of.

The CCMP has Medical Conditions Management, Neurological Conditions Management, Orthopaedic Conditions Management and Gynaecological Conditions Management within this programme.
PROGRAMME HIGHLIGHTS

The Care Team for the management of each chronic condition will consist of the​

The consultant in the concerned speciality;​

Your family doctor;

The physiotherapist;
-
the fitness trainer; and
-
the nutritionist.

The specialist consultant will be your nodal doctor;

You will have initial individual consultations with all of them which will involve detailed history taking, examination and assessment of your chronic condition.

The consultant, in discussion with the others, will prepare a long term treatment plan for you involving medication, diet and nutrition, and any other relevant procedure. This will be explained in detail with you and your caretaker/ close family member.

You will be called for monthly evaluations to the Clinic to monitor and assess your progress.
